Section 2703. State Option to Provide Health Homes for Enrollees with Chronic Conditions
- Establishes a State plan amendment option to provide for medical assistance to eligible individuals with chronic conditions who select a designated provider, a team of healthcare professionals operating with such a provider, or a health team as the individual’s health home for purposes of providing the individual with health home services.
- Defines “health team” as the meaning given to such term in section 3502 of the Act. [The teams in Section 3502 may include pharmacists.]
- Directs the Secretary, no later than January 1, 2014, to survey States that have elected this option and report to Congress on the evaluation and assessment.
- Beginning January 1, 2011, a state may choose to provide health homes.
- Beginning January 1, 2011, the Secretary may award planning grants to States for purposes of developing a State plan amendment.
- No later than January 1, 2017, the Secretary must report to Congress on the evaluation and assessment of the States that have elected this option.
- No later than January 1, 2014, the Secretary must survey states on the impact of this option.
